60/40 Independent Contractor Agreement

 

You: 60%, Wellness Center: 40%

Cap for hours scheduled, not booked:

0-9hrs/wk, no more than $180 monthly.

10-19hrs/wk, no more than $230 monthly.

20-29/wk, no more than $280 monthly.

30-39/week, no more than $330 monthly.

40+/wk, no more than $380 monthly.

 

Included in 60/40:

-MINDBODY software

-Online scheduling (clients love this. Option not available if using your own prices and promotions unless you are bringing a new therapy to Healing Art.)

-Google Adwords advertising

 

Add-ons per treatment/session:

Laundry $6

Cream, oil, Crystal Springs water, hot stones, hot/cold packs & Chinese cups $4

Or pay $0 per massage for add ons and no cap on 60/40.

You may use our tables or bring your own. We won't charge for table rental. We prefer to not move tables around if sharing rooms with other therapists.

 

Everyone will acquire their own credit card processing and handle their own change/cash. We like intuit processing for our mobile phones or tablets but their are several others to choose from. At the end of the month we will inform you of the amount owed toward rent due on the 1st and no later than the 5th. It is your responsibility to with hold the 40% from your income for this.
